Director's record of applicants for relief

44:4-50. Director's record of applicants for relief Directors of welfare shall keep a record which may be by card index and which shall state in respect to all applicants for relief: a. The name, age, sex and residence; b. The number and names of children and their ages; c. The time and place of last employment, and the family income; d. Whether citizen or alien and the place of nativity; e. The place of abode for the ten years preceding the application for relief; f. The causes direct and indirect which operated to make relief necessary so far as can be ascertained; g. The relief or aid given, and such relief as may have been or is being provided by all organizations as ascertained; h. The names of the director or deputy and helper having particular knowledge and charge of the case, and of witnesses of the fact with their addresses; and i. The names of those responsible by law for the support of the poor person and of any relative agreeing or likely to agree to contribute in whole or in part to, or assist in, the support of the poor person.
