Reports by physicians of cerebral palsy cases

26:5A-1. Reports by physicians of cerebral palsy cases Every physician shall report each case of cerebral palsy now under his care, if his patient is under eighteen years of age, and all new cases of cerebral palsy, regardless of the age of the patient. The said reports shall be made to the local boards of health having jurisdiction over the territory within which the patients reside or are being treated. The State Department of Health shall prescribe and furnish the forms on which the reports shall be made, and shall make and promulgate regulations pertaining thereto. L.1950, c. 28, p. 58, s. 1.
